Wootton Vale and Shortstown Surgery is a forward-thinking and innovative practice looking to recruit a health care coordinator to join our dynamic team on a full or part-time basis. Although this is a permanent position, we will consider flexible contract options. Part-time hours will need to be a minimum of 28 per week.
We are looking for an organised, self-motivated, and care-driven individual to join our dedicated clinical liaison team. We seek an individual who can demonstrate a talent for helping our patients achieve the best clinical outcomes and who is looking to build a career in the healthcare sector. Main duties of the job
* Manage referrals via the appropriate system for clinicians.
* Follow up referrals if required.
* Coordinate the referral process and ensure all referrals are received.
* Coordinate birthday QOF reviews for all long-term conditions.
* Generate ICE forms for all blood tests.
* Generate invites and coordinate booking of appointments for these reviews.
* Arrange home visits for housebound patients.
* Maintain obesity register.
* Report regularly to immunisation lead if these targets are lowering.
* Monitor the non-attenders and follow all DNA pathways as per protocol.
Some flexibility is required as you will sometimes need to work additional or alternative hours to cover for colleagues.

Person Specification
Experience
* Experienced GPA in a GP practice.
* Experienced in medical administration with primary care experience.
* Adaptable, friendly, and self-motivated, with strong administration, time-management, and organisational skills.
* Excellent communicator, who works in a confidential and discreet manner and can think on their feet.
* Able to multi-task, work hard, and maintain a high level of attention to detail when under pressure.
* Able to work as part of a multi-disciplinary team, as well as being comfortable working independently.
* Previous experience using IT systems in primary care or hospital secretarial setting is essential for equipping you with the skills and experience needed for the role.

£12.50 an hour rising to £15.00 an hour dependent on experience.
