We are looking to recruit a Band 6 Community Specialist Respiratory Healthcare Professional based at The Coley Clinic in Reading. You will lead on raising awareness in chronic obstructive pulmonary disease (COPD) management and will have the opportunity to develop existing knowledge and skills alongside understanding service needs and organizational strategies from a management perspective.
You will oversee the Berkshire West Community Based Pulmonary Rehabilitation Programme, part of the Community Respiratory Service, within the Cardiac and Respiratory Specialist Services. You should have experience in respiratory care and pulmonary rehabilitation, along with experience in old age medical conditions and supervision of junior colleagues and students.
As part of the Community Respiratory Team, you will use clinical reasoning skills, clinical research, audit, and data collection to provide support and advice in a community setting. You will work with the Community Based Respiratory Team, other care teams, social services, and physiotherapy managers. Main duties of the job
1. Work as part of an integrated team leading the care for those with chronic respiratory disease.
2. Work as part of a 2 Hour Rapid Response Service to reduce the need for acute service intervention, supporting early discharges, and empowering patients to manage their condition.
3. Be responsible for running the Berkshire West Community Based Pulmonary Rehabilitation Programme.
4. Work within an integrated service to improve the lives of patients with respiratory conditions and heart failure.
5. Participate in the Home Oxygen Assessment and Review Service (HOS-AR) to assess patients with an oxygen prescription.

Person specification
Education / Qualifications / Training
* HCPC Registered or RGN
* Educated to degree level or above
* COPD respiratory diploma/degree or other recognized respiratory courses
* NMP qualification or willingness to work towards this
Previous Experience
* Relevant experience in respiratory medicine
* Experience supervising juniors/assistants/students or experience as a clinical educator
* Experience of working within the NHS
* Experience of delivering Pulmonary Rehab
Knowledge, skills and abilities
* Clinical knowledge and skill in relevant area
* Ability to work independently with good use of initiative
* Ability to communicate complex and sensitive information
