Heathside Assessment Pathway Service
Salary: Grade 5, £17,562 - £19,340 per annum. £13.47- £14.83 per hour (pro rata based on 37 hours per week)
Hours: 25 hours per week
Contract: Temporary until 31st December 2025

Your role:
You will provide support to adults in a therapeutic setting and a reablement service, giving people the opportunity to maximise their skills, talents, and aspirations. You will carry out therapeutic activity in accordance with agreed assessment plans to encourage independence and self-management. You will support the seven-bedding setting allowing adults who have been medically optimised to reach their full potential. You will assist your team leader in identifying appropriate exits from the service and current and future care needs, encouraging individuals to maintain independence and self-management.
About you:
You will provide positive outcomes for people with disabilities and have the ability to work independently or as part of a team. You will display values and compassion when supporting people to maximise independence, and have the ability to take positive risks in enabling people to maximise skills and independence. You will support the team leader in finding solutions to maintain people’s independence.
