Responsibilities Respiratory Nurse Specialist Permanent up to 37.5 hours per week Band 6 £37,338 - £44,962 An opportunity has arisen for an experienced nurse who is passionate about providing high standards of care and management for patients diagnosed with long term respiratory conditions. You will work with our Locala Respiratory Specialist team to deliver tailored education for the development of self-management skills for patients and their carers’ You will also provide management for the avoidance of unplanned and unnecessary Gp contacts and Hospital admissions as well as using skills to provide specialist respiratory assessment of patients as part of Virtual Ward discharge support The role will incorporate advice & education within the primary and secondary care environment and provide a resource for health care professionals and patients, providing both formal and informal education and training. You should be educated at or working towards degree level qualification, have post registration experience in specific respiratory conditions, worked with patients and significant others to support personal change management strategies and have experience/good knowledge of working in primary care. If you have some experience and have an interest in respiratory conditions, we would look at supporting a developmental role for a Band 5 nurse who is dedicated in providing management and supporting patients with their long term respiratory condition to work towards Band 6.
