We are looking to grow our Lymphoedema Team with a caring and dynamic individual who enjoys working in the community.
Would you like to provide care for patients with Lymphoedema and chronic oedema as one of the only providers in the area you would be welcomed as part of this integral team.
Do you enjoy managing your own caseload and working autonomously in a range of settings; assessing complex needs and providing high level care and support to those receiving palliative care or those who present with chronic conditions?
Would you like the opportunity to support the Lymphoedema Nurse Specialist and develop your skills by deputising in their absence?
If not a Lymphoedema specialised nurse, would you like the opportunity to complete a fully funded diploma in Chronic Odema Management?,
* To maintain expert levels of clinical competence in regard to lymphoedema management
* To ensure a high standard of clinical care based on available evidence, national guidelines and professional standards, supporting more junior staff to do likewise.
* To recognise the need for medical intervention and refer accordingly
* To facilitate referral to other members of the health and social care team as appropriate
* To be responsible for a caseload of patients, including those with a complex presentation, undertaking a comprehensive assessment, formulating individual management and treatment plans, and, using a wide range of treatment skills and options to provide a specialised programme of care.
* To provide spontaneous and planned advice, teaching and instruction to relatives, carers and other professionals, in order to promote understanding of the aims of lymphoedema management, and to ensure a consistent approach to the care of the patient.
* To provide choice for patients as to how they receive their care, e.g. use of digital technology/on line platforms.
If this all sounds good to you and you are a proactive, enthusiastic team player, keen to champion our organisational vision to positively inspire a future in which everyone within our community facing life limiting illnesses, regardless of cause, receives the best possible palliative and end of life care and their families and carers receive the support they need, please get in touch.
