An exciting opportunity has arisen to recruit a Band 5 Registered Nurse to work with our existing multidisciplinary team on a permanent basis.
Working in the intermediate care setting, you as a registered nurse will have a great opportunity to support the intermediate care pathway as a developing service, also advancing yourself as a registered professional due to the ongoing development of the unit. As part of this post, you will be expected to be flexible in your working pattern to cover a 7-day, 24-hour service.
The service focuses on a short-term assessment period with intensive therapy input for people requiring extra support and rehabilitation to return to their place of normal residence. The Acorn Intermediate Care Unit provides this care for both hospital and, at times, community services.
As part of the rehabilitation team, you will have excellent organisational and time management skills. You will communicate with a wide range of professionals and others about complex issues, or issues relating to discharge planning, be flexible and dynamic, and have a positive approach to enhancing service delivery and implementing NHS Frameworks and Quality Guidelines.
Main Duties of the Job
As a Registered Nurse, you will be responsible for supporting your Manager in ensuring that our service users' needs are met through the delivery of high-quality care. The responsibility of the postholder is to take a role in the coordination, planning, and delivery of individual care provided by the multidisciplinary team.
The post holder will be required to have effective communication skills and work collaboratively in conjunction with the multi-disciplinary team to provide the relevant population of Barnsley with the highest standard of care for an integrated care pathway to enable rehabilitation from hospital to the community.
The post holder will take a lead role in promoting clinical excellence through their practice, participating in evidence-based practice.
Maintain a high standard of clinical work when carrying out nursing procedures. Responsible for the safe custody of medication, including checking and administration. Ensure compliance with our clinical governance framework; you will motivate colleagues, champion service user safety and dignity, and drive continuous improvement.
