Job Summary
We are seeking a dedicated and knowledgeable Staff Pharmacist to join our healthcare team. The ideal candidate will be responsible for ensuring the safe and effective use of medications, providing exceptional patient care, and collaborating with healthcare professionals to optimize therapeutic outcomes.
Responsibilities
1. Review and interpret physician orders to ensure appropriate medication therapy for patients.
2. Dispense medications accurately and provide clear instructions to patients regarding their use.
3. Monitor patient responses to medications and make recommendations for adjustments as necessary.
4. Educate patients and healthcare staff on medication therapies, potential side effects, and interactions.
5. Collaborate with interdisciplinary teams to develop comprehensive care plans for patients.
6. Maintain accurate records of medications dispensed and ensure compliance with regulatory standards.
7. Participate in quality improvement initiatives aimed at enhancing patient safety and care outcomes.
Minimum Requirements
1. A Doctor of Pharmacy (Pharm.D.) degree from an accredited pharmacy school is required.
2. Pharmacist license in the province of Ontario.
3. Previous experience of at least one year is a must.
4. Strong communication skills and a commitment to providing high-quality patient care.
