Job summary A rewarding opportunity for a Health and Wellbeing Coach to work in practices across North Solihull providing one to one and group support to patients with low level mental health support needs. The role will be practice based and you will form an integral part of the Primary Care team, employed by Gateway Family Services, a Community Interest Company and leading provider of health and wellbeing services in Birmingham and Solihull. In addition to Primary Care colleagues, you will also work closely with the wider Gateway team comprising of Social Prescribers and Lifestyle Advisors, working collaboratively to provide best fit support for patients and supporting each other as peers. Main duties of the job As a Health and Wellbeing Coach you will rpvide one to one and group based support to patients referred by GPs and other Primary Care staff. You will rovide low intensity interventions to support patients with self management and recovery from lower level mental health issues. Support will focus on motivational interviewing, goal setting and action planning with support spanning a shorter time frame of around 6 appointments, maintianing a caseload and accurate redord keeping. You will work as part of practices teams across a designated group of practices, forming part of a team of Health and Wellbeing Coaches working across North Solihull.
