12 Month FTC for Maternity Leave Cover
22.5 Hours Per Week (Weekdays only)
St Luke's Hospice is looking for someone who has experience working within health and social care, who has a passion to use this knowledge to enhance the lives of our Palliative care patients and their significant others.
As our Integrated Care Facilitator, you will use your specialist knowledge of the health and social care system to support your clinical colleagues within the Hospice, primary care, and wider healthcare providers. You will work directly with patients and their significant others in the communities where they live, helping develop a deep understanding of the many local initiatives which can support them in a more creative way.
You will be pivotal in helping us develop and evaluate this exciting role, and your views and experience will be critical in helping us be the very best at what we do.
It is your values that are most important to us. Experience of working within the clinical setting and/or specialist palliative care would be an advantage but not essential. Training will be given to the right individuals. Knowledge of care provision across Plymouth and South Hams and West Devon would be a particular advantage.
St Luke’s is a friendly, dynamic, and progressive organisation, where innovation and a ‘can-do’ ethos are championed. We want our team to represent the diverse community we serve and have a strong track record in supporting flexible working and developing people. Our One Team approach values everyone’s contribution and we look forward to welcoming you to the team.

Why work for St Luke’s?
We value all our people - not only our specialist doctors, nurses, and social workers, but also our colleagues behind the scenes, from our maintenance and catering staff to our fundraisers, administrators, educators, and IT team. It takes a vast variety of skills, experience, and knowledge to keep our vital service running smoothly.
As an Employer of Choice, we offer our staff a wide range of benefits and rewards, including generous annual leave and pension schemes, as well as a fantastic health scheme to help towards the cost of eye tests, dental care, and physiotherapy.
While working with our hospice is hugely rewarding, we recognise it's work that can be challenging emotionally. That's why, when you join us, we'll be here to support you all the way.
Don't miss out on the opportunity to join our friendly, award-winning team.
