An exciting opportunity has arisen for an enthusiastic and creative Band 5 Physiotherapist to join the East Community Rehabilitation team.
We are looking for a flexible, reliable person who enjoys working as part of a wider multi-disciplinary team.
The community post will cover community rehabilitation patients with a complex and varied caseload, covering a wide variety of conditions including orthopaedics, MS, MND, Parkinson's, amputees, falls, and frailty. Every visit and treatment plan is different, bringing variety and challenge to each working day. The community area covers the Caradon area. There will also be opportunity to gain some inpatient experience at Launceston and Liskeard Community Hospitals.
This post is currently Monday - Friday, although there may be scope to extend this to 7-day working in the future.
Main duties of the job
1. Assessing new patients on our community waiting list in the patient's homes, including residential and nursing homes. There may also be some opportunity to bring those patients into a hospital outpatient setting to be seen if appropriate.
2. Setting treatment goals and following out treatment plans.
3. To in-reach into community hospitals to facilitate early discharge into the community and to continue rehabilitation in the community setting.
4. Liaising with the wider multidisciplinary team to enable the patient to reach their goals.
5. To support the orthopaedic team with complex discharges post-operatively.

* Degree or Diploma in Physiotherapy
* HCPC Registered
* IELTS unless qualification taken in English
* Experience of working within a multidisciplinary team and an understanding of roles
