The Rapid Transfer Service (RTS) is responsible for the complex discharges from East Kent Hospitals, Pathway 1, Pathway 2 and Pathway 3. The RTS are based on each hospital site: William Harvey Hospital (WHH), Ashford, Kent and Canterbury Hospital (K&C), Canterbury, and Queen Elizabeth Queen Mother Hospital (QEQM), Margate.
Main duties of the job
The post holder will work as a key member of the RTS, whose aim is to ensure the timely, effective, appropriate and safe discharge of patients requiring supportive discharge from the acute into a community setting. The role involves liaising with hospital, community, GPs, and private facilities to promote safe and robust discharge/transfer of care.
A clinical background for this post is essential.

Person Specification
Qualifications
* Registered health care professional (NMC or AHP registration).
* Minimum of 2 years experience at a Band 6 level and experience of working in an acute and community setting.
* Health-related degree and evidence of relevant postgraduate development at Level 6.
* Evidence of Post Graduate Specialist qualifications relevant to the area of practice (examples: non-medical prescribing, mentorship, coaching, clinical expertise).
* Management or supervisory qualification/programme of study.
* PTLLS/DTLLS, PGCE, or Certificate in Education.
* Working towards Master level study.
Experience
* Minimum of 2 years' experience in the area of practice.
* Experience of leading and managing teams.
* Experience of developing policy and service development plans with practical experience of change management.
* Practical experience in managing health-related projects/complex caseloads.
* Supporting others (mentor/educator/supervisor) experience.
* Experience with the use of Microsoft Office and a variety of IT packages (E-Pay/E-roster/AT-P/CIS).
Special Knowledge and Expertise
* Understanding of National Service Frameworks and NICE guidance relevant to care setting and its application across health care services.
* Evidence of continued relevant professional development and implementation in practice within the last 12 months.
* Proven ability to develop effective partnerships to achieve practical outcomes across organisational boundaries including statutory organisations, external agencies, community, and voluntary organisations.
* Supporting others (mentor/educator/supervisor) experience.
