Job summary
1. Participate in the triage of TVOS/Lymphoedema/Hub Dressing referrals and process within set Key Performance indicators
2. Work as part of a team delivering patient centred, evidence based holistic care within the specialist area of tissue viability, wound care and Lymphoedema.
3. Work with primary, secondary care and nursing home staff and the multi-disciplinary team offering advice and educational input
4. Provide care provided in a variety of settings clinics, Wards, Hospice, Care Homes, Support Living and patients own homes.
5. Engage with partner organisation regarding promoting the service and develop effective pathways for patients
Main duties of the job
6. To be responsible for the assessment, planning, implementation and evaluation of individualised evidence based care with minimal ongoing supervision of senior qualified clinician
7. Demonstrate a positive and realistic attitude to the patient and their carers with cancer related and non cancer related lymphoedema to ensure that the physiological, psychosocial, spiritual and cultural needs of the patient and family are met
8. Assess the informational needs of the patient, use appropriate materials to ensure optimum understanding of the causes of the condition, its management and anticipated outcomes of treatment
9. Assess, manage, re-assess and negotiate with patients with tissue viability and lymphoedema needs, using reflective practice and clinical reasoning to identify their physical, psychological and psychosocial needs and identify treatment goals and plans. This post will have access to senior support for advice for more complex patients.
10. Promote the health of patients and assist in the prevention of secondary complications of patients with lymphoedema and tissue viability problems, through enabling and encouraging independence, provision of support, facilitation of advice and information and education and training
11. Liaise with and advise Community Nursing staff/Care Home staff and other healthcare professionals to provide specialist assessment and personalised care planning and intervention to all patients referred to the service
