Job responsibilities Role Description To address the ever-increasing demand for primary and secondary care services and to better support individuals within their own communities, there is wide recognition of the need to embrace new ways of working to ensure all patients receive the right care, in the right place, at the right time. Over the last few years, Jurassic Coast PCN has embraced several new and innovative ways of delivering improved care to its patient population. The role of PCN Administrator will be to support the management and PCN teams with their caseload, compliance, and daily tasks organisation. Administrators will support their PCN colleagues to provide an integrated and synchronised approach to care, ensuring that patients receive the right level of support from our PCN teams by coordinating patient appointments and supporting with day-to-day administration. Our Primary Care Network is committed to developing a multidisciplinary workforce, you will work with different staff members including management, clinical and other administration colleagues. You will provide administrative support to the Clinical Director, Digital and Transformation Lead, PCN Deputy Manager as well as our Pharmacy, Frailty and Social Prescribing Teams. You will closely work with our member practices to develop and support service improvement in line with local and national priorities. Building strong relationships with primary care colleagues and local partners to proactively support and drive change will be a key element of this role. Key Responsibilities Administration Run regular reports utilising the clinical system, including both proactive reports to identify patients who may benefit from the PCN services, as well as regular reports to indicate PCN progress. Liaise with practice managers regarding room availability for PCN staff and set up and amend clinic rotas. Attend and take minutes for a variety of PCN meetings and distribute to all members in a timely and accurate manner. Generate documents and action plans resulting from meetings and distribute to attendees in an accurate and timely manner, including following up and monitoring progress against required actions. Liaise with members of the network and arrange meetings as required and collate and distribute meeting papers to all relevant attendees. Collate and edit newsletters and publications for the PCN and patient groups. PCN event management, including training and patient events. Maintain and monitor referrals and caseloads for the PCN teams, including preparing required documents and sending out communications as required by Team Leads and Management. Manage the PCN inbox and incoming phone calls, responding to enquiries where possible and triaging and escalating as required, including diary management for teams where appropriate Assist in the revision and maintenance of filing and administrative systems so that written and digital information is easily accessible and secure. Carry out additional appropriate administrative and operational duties as required by members of the PCN. With the support of IT staff, administer, search and report on a range of IT systems including but not limited to SystmOne, Microsoft Office Suite and Dorsets Population Health Management Dashboard. Maintenance of the PCN intranet, website, and social media. Ensure PCN staff have necessary access to practice clinical and administrative IT systems. Ensure key documentation such as consent and feedback forms are obtained and filed appropriately. Respect for Patient Confidentiality The post-holder should always respect patient confidentiality and not divulge patient information unless sanctioned by the requirements of the role. Special Working Conditions The post-holder is required to travel independently between PCN sites, and to attend meetings etc. hosted by other agencies. Job Description Agreement This job description is intended to provide an outline of the key tasks and responsibilities only. There may be other duties required of the post-holder commensurate with the position. This description will be open to regular review and may be amended to take into account development within the PCN and member practices. All members of staff should be prepared to take on additional duties or relinquish existing duties in order to maintain the efficient running of the PCN.
