Job summary
We are seeking a compassionate and dedicated Assistant Practitioner to join our Community Mental Health Team for Older Persons at the Sophie Centre, Julian Hospital, Norwich. In this role, you will support older adults experiencing mental health challenges, working alongside a multidisciplinary team to deliver high-quality, person-centered care.
Main duties of the job
Key Responsibilities:
1. Assist in the assessment, planning, and delivery of care to older persons
2. Work collaboratively with nurses, psychologists, and other healthcare professionals
3. Provide emotional support and practical assistance to service users and their families
4. Contribute to the overall wellbeing of individuals in the community
Requirements:
5. Experience in mental health or elderly care is desirable
6. Excellent communication and interpersonal skills
7. Ability to work autonomously and as part of a team
8. Relevant health or social care, foundation degree qualification required
