Main duties of the job
As a Staff Nurse, you will be responsible for providing care to people admitted to our ward for rehabilitation. We work closely as a multi-disciplinary team. Our focus is promoting the comfort and wellbeing of patients, enabling them to live as independently as possible. As a team, we encourage ongoing learning and development through reflection and clinical supervision, which we try and embed in our daily practice. Our nurses are required to facilitate the day-to-day running of the ward, administering medications, working alongside all members of the MDT as well as patients, their families, and carers to provide 24/7 rehabilitation to enable the highest level of independence and quality of life. Our nurses are required to attend meetings as needed, such as MDT and CHC or Discharge Planning.

Job responsibilities

* In consultation with senior qualified staff, provide and monitor patient-centred care within Trust agreed standards.
* In collaboration with other team members, organise the reception and discharge of patients ensuring effective communication in the transfer of information.

Qualifications
* RGN/RMN
* NMC Registered
* I understand that I will be asked to provide original certificates to verify my qualifications at the point of offer
* Practice Supervisor and/or Practice Assessor Training.
* Accredited courses in Palliative Care, Tissue Viability, Diabetes, Dementia.
Experience
* Experience gained during pre-registration training.
* Experience gained working in the frailty care setting.
