Clinical Nurse Specialist Role

Salary: £42,000 - £45,000 per annum (dependent on experience) plus £3,000 car allowance
We are seeking a highly skilled and motivated Clinical Nurse Specialist to join our team. The primary objective of this role is to deliver high-quality, patient-centred care to individuals with complex health needs living in their own homes.
The ideal candidate will have a passion for clinical leadership and be able to manage a caseload of complex patients, providing expert clinical advice and guidance to nursing staff and other healthcare professionals.
In addition to their clinical expertise, the successful candidate will possess excellent communication and interpersonal skills, enabling them to build strong relationships with patients, families, and healthcare colleagues.

Key Responsibilities:
* Manage a caseload of complex patients with medium to long-term care needs
* Provide expert clinical advice and guidance to nursing staff and other healthcare professionals
* Develop and implement individualised care plans, promoting independence and well-being
* Conduct comprehensive assessments, identifying patient needs and developing effective interventions
* Maintain accurate records and contribute to quality improvement initiatives
Requirements:
* Registered Nurse with a valid NMC registration
* Post-registration qualification in a relevant specialty (e.g., district nursing, community nursing)
* Experience of working with complex patients and their families
* Strong clinical leadership and management skills
* Excellent communication and interpersonal skills
