Purpose of the role:
Without prior GP referral, First Contact Practitioners (FCPs) consult directly with patients, enabling rapid and accurate MSK diagnosis or providing referral to secondary care for further investigation. The role facilitates streamlined pathways of care by interpreting and acting on assessment findings and management plans. While FCPs are based within a GP Practice, support is provided through a robust network within the organisation, including direct Line Managers, roadmap supervisors, and a wider regional team that is accessible for assistance and support.
Core Responsibilities:
Serve as an expert MSK clinician within primary care, managing patient needs within scope of practice and utilising relevant services to support GPs.
Embed within the PCN, integrating as part of the team, and engaging with all stakeholders.
Deliver programs of supported patient self-management to facilitate behavioural change, optimise physical activity and mobility, and minimise pharmacological interventions.
Uphold company beliefs and ethos as an ambassador for the organisation, ensuring thorough and transparent communication throughout work and with involved teams.
Demonstrate flexibility to work across multiple GP Practices within the PCN contract.
Adapt to changing demands of PCNs and the evolving landscape of primary care.
Track patient appointments to enable the business to demonstrate value to service commissioners.
Ensure comprehensive clinical documentation.
Qualifications and Experience:
Essential:
HCPC Registration.
Bsc Physiotherapy
Minimum of 5 Years MSK experience
Desirable:
Post-graduate / master's level qualification
Roadmap Stage 1 signed off.
Roadmap Stage 2 signed off.
Skills:
Strong communication, organisational and time management skills.
Good IT skills including word processing.
Strong problem solving skills.
Qualities:

Confident in decision making and able to ask for help.
Proactive and self-motivated.
Team player with a positive attitude.
Ability to adapt to a fast-paced and changing environment.
