Job description Overview The Community Nurse plays a vital role in delivering high-quality healthcare services to individuals and families within the community. This position involves working collaboratively with patients to assess their health needs, develop care plans, and provide ongoing support and education. The Community Nurse is dedicated to promoting health, preventing illness, and enhancing the quality of life for the community members they serve. Duties Conduct comprehensive assessments of patients' health status and needs. Develop and implement personalized care plans in collaboration with patients and their families. Provide nursing care, education, and support to patients within community hospital settings. Monitor patients' progress and adjust care plans as necessary to ensure optimal outcomes. Maintain accurate records of patient interactions, care provided, and any changes in health status. Collaborate with other healthcare professionals to coordinate patient care and resources. Educate patients on managing chronic conditions, medication adherence, and healthy lifestyle choices. Participate in community outreach programmes to promote health awareness and disease prevention. Skills Strong time management skills to effectively prioritize tasks and manage caseloads. Excellent communication skills for interacting with patients, families, and multidisciplinary teams. Proficiency in developing and implementing care plans tailored to individual patient needs. Demonstrated leadership abilities to guide patients in their healthcare journey and mentor junior staff.
