Trainee Assistant Practitioner - Special School Nursing
In conjunction with the specialist nurse, provide a specialist school nursing service for children with complex medical needs, liaising and coordinating with all professionals involved in the child's/family care.
Work under the guidance and instruction of specialist Nursing staff to provide personal care and support to children and young people to enable their access to learning. The work may be carried out in the classroom or outside the main teaching area.
The post holder provides education, advice, and support to enhance independence.
The post holder will empower and support families to manage their child's medical needs at home and school, thus avoiding hospital appointments/admissions.
The post holder will support the Special School Nurses by obtaining specialist knowledge in the field.

Main Duties of the Job
1. Undertaking tasks that have been delegated by the nurses ensuring that a child or young person's health needs are met within the school.
2. Act with the Specialist School Nurses as a key worker for children with complex medical needs within the family.
3. As a health professional, become involved with coordinating pathways of care and help develop the service to meet national guidance and standards.
4. Provide education, advice, and support empowering families to manage their child's medical needs at home.
5. As a health professional, assist with the management of health problems arising, to maintain the child in the school setting.
6. Liaison between school and home.
7. Act as a specialist resource by providing education and advice to multi-disciplinary staff.
8. Attend multi-agency professional meetings if required.

14. The SHCSW is responsible for inputting information into the electronic patient record where applicable and also into patients' written records.
