Virtual Ward Frailty Clinical Nurse Specialist
We are seeking a dynamic Clinical Nurse Specialist who has a passion for frailty, to join the Frailty Team based within Same Day Emergency Care at Russells Hall Hospital. The main priority will focus on implementation and operation of Frailty Virtual Ward within the community in Dudley. The successful candidate will join the integrated team in the delivery of quality care for people and will be integral in enabling patients to achieve optimum function and receive appropriate treatment within their own home to prevent unnecessary admission to hospital. A highly focused MDT approach to care is encompassed within the Frailty team and there will be opportunity to complete focused Comprehensive Geriatric Assessment within primary and secondary care. There is an expectation for hybrid working in both Frailty Virtual Ward and close collaboration with Frailty Assessment Unit.
Main duties of the job
This role is hybrid and will require the post holder to work across primary and secondary care. The ability to work independently is essential as the post holders will need to offer urgent care, treat minor illness, give patient advice and offer chronic disease management. You will have the ability to prioritise and triage the needs of patients, making any referrals for further investigations where appropriate in line with local and practice guidelines, whilst ensuring your clinical practice is safe and effective.
You will be supported by the senior clinical team including trust doctors/consultants, dedicated advanced clinical practitioners, therapy staff and senior pharmacists. There will be daily board round meetings held to discuss the patients cared for on the Virtual Wards and a weekly MDT meeting which will be held in Frailty Assessment Unit.

Qualifications & Education
* Registered nurse with NMC
* Educated to degree level
* Health Assessment & Physical Qualification (at a minimum of level 6) or be willing to undertake
* Masters degree in relevant Clinical Practice
Experience
* Experience of working in an acute hospital setting
* Evidence of being able to prioritise workloads and delegate to staff
* Experience of working with people with long-term illness and or older people
* Experience of ward management
Knowledge and Training
* Completion of Tier 3 Frailty competencies
* Evidence of professional development
