The ideal candidate will be an experienced and clinically skilled Practice Nurse able to provide a range of Nursing services, including Chronic Disease Management as well as core Practice Nursing duties.

Main duties of the job
To provide a comprehensive range of nursing services to the whole Practice population, delivering a cost-effective service, maximising access for our patients and Practice achievement in QOF, Enhanced Services and other new contractual incentives as they arise.
The focus of the role encompasses the delivery of evidence-based practice for patients presenting with a long-term condition, the provision of preventative health care to the Practice population and working autonomously within a specialist clinical area.
The post holder will also provide clinical supervision and support professional development for more junior professional and non-qualified colleagues within the Practice Nursing Team.

Job responsibilities
The post holder is responsible for the delivery of general practice nursing to the whole practice population. The focus of the role is both the delivery of evidence-based practice for patients presenting with a long-term condition, and the provision of preventative health care to the practice population.
The post holder will be supported in their role by more senior nurses and the GPs. They will work collaboratively within the general practice team to meet the needs of patients and support the delivery of policy and procedures.
Person Specification
Experience
* Minimum two years post-registration experience.
* Operating without direct supervision following protocols.
* Practice Nurse training.
* Experience of whole systems team approach.
* Delivery of care within a clinic setting.
* Experience of nurse-led management of long-term conditions.
Qualifications
* Registered General Nurse Part 1 NMC Registration.
* Evidence of continuous professional development.
* Clinical supervision training and experience.
