We are seeking experienced physiotherapists to join our rapidly developing stroke service in Carmarthenshire, supporting acute rehabilitation and early discharge from hospital.
If you have a passion for stroke rehabilitation working across the stroke pathway this could be the opportunity for you. You will be based at Prince Philip Hospital, Llanelli supporting people who have predominantly experienced a stroke to return home from our acute stroke units.
The role will aim to deliver rehabilitation in hospital and at home in the local community to support transition from hospital and return to independence. To deliver a person-centred rehabilitation and transition pathway the post holder will work alongside the established acute and community stroke teams (in-reaching and out-reaching) to ensure good continuity of care.
We are advertising for 1 post holder - based in Prince Philip Hospital Llanelli.
Main duties of the job
Successful applicants will have some experience of working with people who have experienced stroke and/or other life changing events and enjoy working as part of a multi professional team within the acute and community setting. You will work alongside acute and community colleagues and services to create a joined up rehabilitation pathway and positive experience for our population.
Whilst the role focusses on stroke, we require therapists who are adaptable to meet needs of patients with multiple and a variety of conditions as well as service priorities. Due to the nature of the role you will need to be able to travel within the County, as well as being flexible to travel across the Health Board on occasions.
The post holder will be required to participate in the provision of on call respiratory service and future developments in 7 day working.
