Job Summary: The role of Advanced Clinical or Nurse Practitioner (ACP/ANP) requires an experienced, autonomous clinician who will provide patient care across a spectrum of needs, including clinical assessment, diagnosis, treatment, and evaluation. Working within professional boundaries, the post holder will demonstrate expertise in clinical decision-making and patient care within general practice, using a holistic approach to treatment and patient education. This role requires the ACP or ANP to act in alignment with the Nursing and Midwifery Council (NMC) standards for advanced practice, which must be maintained and updated as needed. At all times, the post holder represents School Lane Surgery and the NHS and is expected to uphold a standard of professionalism consistent with both organisations' values. Main Responsibilities: Independently assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis in Primary Care, initiating referral where appropriate Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care using evidence-based practice, managing both acute and chronic conditions autonomously Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice Work with patients to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care and patient education Assess, identify, and refer patients presenting with mental health needs Undertake home visits in accordance with the relevant protocols Recognise and work within own competence and professional code of conduct seeking advice when required and referring onto relevant health professionals/agencies Produce accurate, contemporaneous, and complete records of patient consultation, consistent with legislation, policies and procedures Effectively share relevant information and promote collaborative team working. Working to improve the quality of health care responding to local and national policies and initiatives as appropriate Prioritise, organise and manage own workload in a manner that maintains and promotes quality Deliver care according to NICE guidelines, local guidelines and evidence-based care Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation Initiate and participate in the maintenance of quality governance systems and processes across the organisation and its activities Critically appraise and synthesize the outcome of relevant research, evaluations and audits and act on this information in collaboration with colleagues to continuously develop the services Support and participate in shared learning across the practice and wider organisations such as the Primary Care Network Understand and support the identification of vulnerable and abused children and adults and be aware of statutory child/vulnerable patients health procedures and local guidance Delegate clearly and appropriately, adopting the principles of safe practice and assessment of competence to scope of practice and not acting outside of clinical scope and knowledge boundaries Contribute to the practice achieving its quality targets to sustain high standards of patient care and service delivery as well as highlight and work with the team to create opportunities to improve patient care Provide education and training to other staff and students Participate, when required, in the investigation of patient complaints when requested to do so and participate in the identification of any necessary learning brought about through significant events, incidents and near-miss events Foster a culture of professionalism, compassion, excellence and the development of effective team working There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels.
