Community Health and Wellbeing Worker
Location: Penzance – some local travel
Salary: £25,798.50 FTE
Hours: 37.5 hours per week, Monday to Friday
(will include evenings and weekends as necessary)
Contract: Fixed Term – 3 Years

This is an exciting opportunity to be part of an innovative charity, which is leading the way in addressing social exclusion amongst children, young people and their families.
Trelya aims to break the cycle of social exclusion including enabling adults and young people at risk of social exclusion as a result of social and economic deprivation to develop the skills, knowledge, awareness, aspirations and motivation to make positive choices about their lives and their behaviours.
As a community health and wellbeing worker (CHWW) you will be understanding and working in your local community. The outreach work of a CHWW is essential in helping to address the rising inequities in health and social care and in identifying unmet need in your community. The CHWW will help provide a bridge between community, health and care as well as supporting people to access the right services for them.
The CHWW will need to build long-term trusted relationships with a range of householders and develop consistent relationships with the aim of improving the health and wellbeing of each of the individuals you work with. As a CHWW, you will have a level of self-confidence and personal robustness to enable you to work with the community, and to help people overcome their barriers with a coaching approach. You will have insight into the challenges that people can face, which might come from personal lived experience, from volunteering or from other employment. You will maintain a culture of inclusivity and learn to work in a trauma informed way.
This is an exciting opportunity to work in collaboration with organisations across the voluntary sector and health sector, focusing on what matters to people and empowering them to find local solutions, whilst shaping a new role to Cornwall and the Isles of Scilly.
