Role Summary:
The practice is seeking a skilled and compassionate Practice Nurse to join its dynamic team. The successful candidate will play a key role in delivering a wide range of nursing services, including chronic disease management, immunizations, health promotion, and general practice nursing duties.
Key Responsibilities:
1. Deliver high-quality nursing care, including assessment, planning, implementation, and evaluation of patient care.
2. Manage long-term conditions such as diabetes, asthma, and hypertension.
3. Administer vaccinations and immunizations, including travel vaccinations.
4. Conduct cervical screening and other health checks.
5. Support the GP team with minor illness clinics and triage.
6. Provide advice on health promotion and disease prevention.
7. Maintain accurate patient records and ensure all care is documented in line with practice policies.
8. Participate in clinical audits and contribute to the development of practice protocols.
9. Engage in continuous professional development to ensure up-to-date knowledge and skills.
Qualifications and Experience:
1. Registered Nurse with a valid NMC PIN.
2. Previous experience working as a Practice Nurse in a GP surgery or similar setting is preferred.
3. Experience in chronic disease management, immunizations, and cervical screening.
4. Strong communication and interpersonal skills.
5. Ability to work independently and as part of a multidisciplinary team.
6. Excellent organizational skills and attention to detail.
