Senior Commissioner for Integrated and Personalised Care

This post has been developed to lead the integration of health, care and education to better meet the needs of most complex children and young people; creating sustainable integrated systems and processes that can transition into the new Care Alliances and Integrated Care Partnerships.
The post holder will be the key interface with the four local authorities within the BLMK ICB geography working to support and improve the outcomes children and young people within the Learning Disability and Autism program and others with complex needs.
Bedfordshire Luton, and Milton Keynes (BLMK) Integrated Care Board (ICB) serves the people of BLMK, bringing together health and care, wider public sector and Voluntary, Community and Social Enterprise partners to support and improve our resident's wellbeing. The employees of the BLMK ICB work as part of an Integrate Care System, in particular working with the NHS and Social care and underpinned by relationships across the wider public sector.

Main duties of the job
* The postholder will take the lead for the system wide integration and personalisation agenda relating to children and young people with complex needs that spans health, care and education, where a more integrated multiagency approach is needed to prevent/reduce crisis and manage unavoidable crisis to improve outcomes.
* Providing professional expertise in integrated (multiagency) case management.
* Chairing and participating in multiagency panels and decision-making forums as BLMK ICB senior team member.
* Ensure the delivery of National objectives for Learning Disability and Autism Programme and support delivery of emotional and mental health priorities as included in the Core 20 plus 5, SEND, personalisation and positively influence the practice of health and social care professionals to meet the needs of our children and young people across BLMK.
* Work in a matrix with the children and young people strategic commissioning team to influence commissioning and systems intentions targeted objectives, quality improvements and better outcomes.
* Demonstrate effective stakeholder management across different systems and at all levels.
* Work in close partnership with service users, key stakeholders and co-commissioners, Health Watch and other partners from across the Voluntary Social Enterprise community.

As part of the wider Children, Young People & Maternity structure of BLMK ICB, the successful candidate will become an integral part of the smaller Integrated and Personalised Care Team. Job responsibilities
The Senior Commissioner for Children and Young Peoples Integrated and Personalised Care within Bedfordshire, Luton and Milton Keynes Integrated Care Board (BLMK ICB) has been developed to lead on the Learning Disability and Autism Programme to better meet the needs of the most complex children and young people; creating sustainable integrated systems and processes that can transition into the new Care Alliances and Integrated Care Partnerships.
Key Duties and Responsibilities
* The postholder will take the lead for the system wide integration and personalisation agenda relating to children and young people with complex needs that spans health, care and education, where a more integrated multiagency approach is needed to prevent/reduce crisis and manage unavoidable crisis to improve outcomes
* Providing professional expertise in integrated (multiagency) case management
* Chairing and/or participating in multiagency panels and decision-making forums as an ICB senior team member
* The post holder will be expected to work with children and young peoples health, social care, and educational services (including SEND) to co-ordinate packages of care for individual young people, mobilise support within the system and removing any barriers.
* Establish clear policies and processes (where this doesnt already exist) to embed an integrated care system and risk stratification approaches within the children and young people system e.g., continuing care, Section 117 aftercare, Learning Disability and Autism National Program and Mental health escalations (this is not an exhaustive list)
* Ensure the delivery of national objectives for Learning Disability and Autism Program and support delivery of emotional and mental health priorities as included in the Core 20 plus 5, SEND, personalisation and positively influence the practice of health and social care professionals
* Work in a matrix with the CYP strategic commissioning team to influence commissioning and systems intentions targeted objectives, quality improvements and outcomes
* Interface with BLMK transformation programmes to support the delivery of benefits of service improvement at place and scale.
* Identify where transformation, integration and/or investment would expedite delivery of the benefits, promote the development of innovative approaches to service transformation, and develop business cases to support this
* Demonstrate effective stakeholder management across different systems and at all levels.
* Work in close partnership with service users, key stakeholders and co-commissioners including children and young people; and Health Watch and other partners from across the Voluntary Social enterprise community
* The post holder will define current processes, facilitate discussions and advise colleagues as to how best practice might be adopted in the definition of future processes, document those processes as they are agreed and oversee their delivery.
* Take into account the impact of any change the projects will have on the business and oversee handover of any products to ensure full ownership and buy-in within the business. Advocate the projects at senior and executive levels and ensure active engagement and sponsorship within ICB/LA as a whole.
* Ensure that the projects maintain business focus, have clear authority and that the context, including risks, is actively managed in alignment with the strategic priorities of ICB /LA.
* Deputise for BLMK ICB Head of Service for Children and Young People Integrated and Personalised Care, expanding on knowledge, skills and experience with personal professional development

Person Specification
Knowledge, Training & Experience
* Demonstrable experience of working in a large complex organisation.
* Educated to masters level or equivalent level of experience of working at a senior level in specialist area.
* Extensive knowledge of specialist areas, acquired through post graduate diploma or equivalent experience or training plus further specialist knowledge or experience to master's level equivalent
* Evidence of post qualifying and continuing professional development
* Must have an understanding of the background to and aims of current healthcare policy in BLMK and appreciate the implications of this on engagement
* Should have an appreciation of the relationship between the Department of Health, the ICB, Local Authorities and individual provider and commissioning organisations
* Experience of working within the NHS or Local Authorities at a senior level.
* Experience of personalised commissioning for Children and young people.
* Experience of the Learning Disabilities and Autism Programme (Transforming Care).
* Experience of working in the field of complex care.

* Ability to manage competing priorities in a busy and changing environment

* Ability to travel to other sites within the BLMK area
* Able to work outside of traditional office hours on occasion.
Professional Registration
* Registration with a relevant professional body as a nurse, therapist or social worker

* Developed communication skills for delivering key messages to a range of stakeholders both internal and external (including outside the NHS) to the organisation, some at very senior level. Good presentational skills for conveying complex concepts.
