Job summary
Weare looking to increase our GP capacity within amulti-disciplinary clinical team to support the delivery of our Enhanced Accessservices on a Friday evening and Saturday.
The post-holder will have theopportunity to work agreed contracted sessions as part of a job-share rota aswell as providing cover for annual leave and sickness absence. We will alsoconsider applications to join our bank of locums who will have the opportunityto undertake sessions on an ad-hoc basis.
Working autonomously, and in accordance with the PCN DES you will undertake a variety of duties including surgery consultations, telephone consultations and queries. This will include dealing with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all Middlewood registered patients.
The post-holder will work un-supervised, takingclinical accountability for their own work and providing clinical supervisionand support for other members of the multi-disciplinary team during EnhancedAccess sessions.
Main duties of the job
Clinical Responsibilities
1. Undertake a variety of duties including face to face consultations, telephone and online consultations and queries, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.
2. When required, during sessions in core hours, support the Middlewood Immediate and Necessary Team (urgent care) and being part of our rota to support our clinical reception triage function.
3. Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
4. Assess the health care needs of patients with undifferentiated and undiagnosed problems
5. Screen patients for disease risk factors and early signs of illness
6. In consultation with patients and in line with current practice disease management protocols, develop care plans for health and social well-being
7. Record clear and contemporaneous consultation notes to agreed standards
8. Collect data for audit purposes
9. Compile and issue computer-generated acute and repeat prescriptions
10. Provide support, informal training and advice to other members of the team
11. In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
