Are you dynamic, motivated, and have a passion for promoting independence? We are looking for an Assistant Practitioner to join our large, well-established and friendly team. Our offices are shared with District Nurses, Social Workers, and Urgent Community Response, allowing a high level of integrated working to the benefit of our patients.
Our caseload covers a variety of rehabilitation needs, from post-op rehab to long-term condition management, falls prevention classes, and same-day equipment assessments to keep patients safe at home. The successful applicant will be responsible for their own caseload, which is supervised by our qualified Physiotherapists and Occupational Therapists.
Applicants will have completed the Trainee AP course and will require sound moving and handling skills, good problem solving, prioritisation, and excellent communication. You will need a driving license and access to a vehicle to be able to complete your community visits.

Job responsibilities

Excellent IT skills are required to coordinate the smooth running of the service and utilise the benefits of our online patient record system, MyCare.
Working Pattern: The successful candidate will complete the majority of shifts from 8:30 AM to 4:30 PM and will be entered into the team rota for late shifts (10 AM - 6 PM or 12 PM - 8 PM) and weekends as part of supporting our urgent work to keep patients safe in their own homes.

Salary: £26,530 to £29,114 per annum pro rata.
