Rehab First is one of Ontario's largest multidisciplinary assessment and treatment organizations, numbering approximately 120 professional staff supported by our MEDNet roster and administrative team. We are currently seeking a Full-Time Treatment Coordinator to work on-site from our London office, located at 1599 Adelaide St. N., Suite 201, London, ON N5X 4E8 to support our expanding Treatment Coordinator Team.
Key Responsibilities:
* Intake of clients into Rehab First for assessment and treatment services
* Advocating for treatment and related services
* Making treatment recommendations for clients
* Interacting with clients to identify their specific assessment and treatment needs
* Working with clients to ensure access to the assessment and treatments required given their illness or injury
* Developing assessment and treatment plans with clients, obtaining third-party approval when necessary
* Assisting with client file reviews
* Cultivating referral customer relationships with insurers, lawyers, employers, physicians, and psychologists
Key Qualifications:
* Registered Nurse, Kinesiologist, Physiotherapist, or Occupational Therapist with proof of registration
* Valid CPR certificate
* Superior communication, planning, problem-solving, time management, and documentation skills
* Excellent customer service skills
* Ability to work collaboratively as a member of a multidisciplinary team
* Self-motivated, with demonstrated ability to manage a busy and varied schedule
* Completion of a satisfactory Police Vulnerable Sector Check
