Job summary
We are seeking autonomous, innovative, practitioners, who would like to work within one of our integrated Community Mental Health Team (South and West) in Bradford. As a service we have developed innovative ways of delivering recovery focused intervention to support people in their individual recovery journey.
The team comprises of Doctors, Nurses, Psychologists, Social workers, Occupational Therapists, support and peer workers delivering a recovery focused service based upon on our Care Closer To Home initiative. The ideal candidate will have a dynamic approach to working as part of the MDT.
The post holder will work directly with service users who have severe and enduring mental health needs, to support them in their recovery, acting as Care Co-ordinator. You will have excellent communication skills and an enthusiasm for service development.
Applications are invited from those who are qualified Band 5 Registered Mental Health Nurses, Social Workers or Occupational Therapists.
Main duties of the job
Responsible for the delivery of community mental health care to a caseload of clients in accordance with the objectives of the Care Trust and within team operational policies.
Ensure implementation of all Trust Policies and Procedures as required.
To receive and accept referrals for the service and initiate, lead and co-ordinate their assessment, making referrals to other professionals as required.
To manage a personal caseload of clients.
To work as a Care Co-Ordinator responsible for assessment, planning, intervention and evaluation of care for a designated client group. Undertake accurate, timely and skilled assessments and ongoing reassessment of client's condition, taking into consideration all social, physical and environmental factors.

Essential
1. Registered Mental Nursing Certificate (RMN)
2. Current Registration on part 3 or part 13 of UKCC register.
Desirable
3. Evidence of post registration training in Mental health needs/ challenging behavior
4. Diploma in Nursing
5. FLiCP/SLiP (or equivalent)
Experience
Essential
6. Experience of working, or placement in a Community Mental health setting
Desirable
7. Experience in relevant area of Mental Health Nursing.
