What the Job Involves Providing social work services within a community mental health setting. Collaborating with a multidisciplinary team to assess needs and establish service eligibility. Managing, monitoring, and reviewing care plans liaising with various agencies to meet assessed needs. Coordinating care packages according to Council’s Financial Regulations. Utilizing information management systems to record and retrieve required information. Participating in safeguarding adult enquiries and multidisciplinary team activities. Undertaking training as required, including training as an AMHP and Best Interest Assessor. Staying updated on legislation, government policies, and guidance relating to mental health and vulnerable persons.
