Community Link Worker - West London
A Community Link Worker is required to work on a rolling contract for a charity based in West London.
The Community Link Worker should have knowledge of their community and the variety of services available there. They should be committed to applying their knowledge to help individuals in their local communities obtain the support they require.
Link Workers aim to make a person's path to improved wellness as straightforward as possible by collaborating with clients, medical professionals, volunteers, and service providers.
Responsibilities:
1. Develop, implement, and evaluate wellness action plans that include one-on-one and group sessions in a coordinated manner.
2. Assess individuals' needs, identify outcomes, and determine the type of support required to overcome obstacles and achieve results.
3. Conduct, evaluate, and update risk assessments related to clients, operations, and the environment.
4. Identify current community resources that can help individuals reach their wellness goals, such as healthy lifestyle options, sports, arts and culture, and statutory organizations, and monitor positive engagement.
5. Monitor individuals' well-being post-program and identify potential risks to maintaining their level of wellness.
6. Collaborate across health and social care sectors with stakeholders, attend relevant meetings, and communicate regularly with social care teams assigned to caseloads.
Experience Required:
1. Experience in case management-based interventions for mental health and wellness.
2. Work experience in primary or secondary health and social care settings.
3. Knowledge of supporting clients facing complex challenges in health and social care environments.
4. Excellent planning skills, well-organized, and able to meet goals.
5. Knowledge of mental health, learning disabilities, special educational needs, inclusivity, and anti-stigma initiatives.
