This is a request for an agency Personal Advisor working on Stockport Leaving Care Team.
Main Purpose of the Job:
To work with young people aged 16 years – 25 years who are or have been looked after or accommodated by the Local Authority in order to plan their transition to adulthood and assist them to live an independent life in their community.
Summary of responsibilities and key areas:
1. To carry out the statutory responsibilities and duties for looked after children in line with the Children Act 1989, Children (Leaving Care) Act 2000 and related regulations.
2. To manage a caseload and keep accurate case records of work with young people and undertake statutory recordings.
3. To contribute to Pathway Plan Assessment and provide ongoing assessment with young people to inform Pathway Planning.
4. To undertake assessments of young people resuming or wishing to resume education/ training or presenting with other support needs post 21 years.
5. To work with young people to promote welfare and ensure that their views and wishes are taken into account when undertaking assessments, developing Pathway Plans and reviews.
6. To provide practical advice, assistance and support to young people.
7. To identify packages of support for young people living in the community and supervise and monitor the delivery of tasks in accordance with the pathway plan to meet young person’s needs.
8. To prepare and deliver Pathway plans with young people.
9. To develop and maintain relationships with young people who are or have been looked after and promote regular contact and provide keeping in touch visits.
10. To promote the involvement and participation of young people in all aspects of service delivery and pathway planning.
11. To prepare reports and present at 16+ multi-agency accommodation panel, Child protection conferences, planning meetings, multi-agency adults at risk (MAARS) panel and court as required.
12. To empower young people and act as an advocate promoting young people’s independence.
13. To work in partnership and liaise with a range of agencies including; Early Help & Prevention Service, Education and Careers Advice Service, Police, YOS, Health, Housing, Higher/Further Education providers and others as appropriate.
14. To ensure that young people receive appropriate financial support and work with young people to develop their financial management skills with a view to them becoming responsible for their own budget.
15. To work with Unaccompanied Asylum Seeking Children in line with Home Office Guidance and adopting triple planning within the Pathway Plan.
16. To have a working knowledge of community resources available to parents and carers and introduce them to these as appropriate.
17. To promote the Care Leaver Forum, mentoring scheme and other support services for young people.
18. To provide advice, guidance and support to social workers preparing looked after children aged 16+ to prepare for transition to adulthood. Being involved in delivering briefing and training as required.
19. To be part of a duty rota providing a duty service to young people.
20. To attend and participate in supervision sessions, team meetings and training programmes in accordance with Stockport Policy and Procedures.
21. To undertake such other duties appropriate to the post as may be assigned by the Team Leader.
