Job summary We are looking for a Nurse/Clinical Practitioner, on a permanent basis, to do 24 hours per week on a Monday, Wednesday and Thursday (8 hours a day). You will be based at Clifton Road Surgery, situated within the centre of Rugby. We are a large and forward looking training practice responsible for approximately 15,000 patients. Main duties of the job We invite applications from suitably qualified Practitioners with skills to clinically assess, diagnose, refer and manage patients with a range of conditions. The ability to work independently and to demonstrate critical thinking in the clinical decision-making process is required and to take lead responsibility for the care of patients as well as working as part of a team. Ensure that patients receive high quality clinical care, delivered in a timely manner. Supply and administer medicines as indicated to address patient need. Ensure complete and accurate documentation of each and every patient contact. Work as an autonomous practitioner and as part of multi-disciplinary and multiagency teams in order to ensure patients needs are met. Refer patients directly to other services/agencies as appropriate in line with Practice policy. Work with multi-disciplinary team within the Practice to promote integrated and seamless pathways of care.
