This post is integral to the NHSE future plan for general practice which encourages the development of this role and will partake in the national learning and knowledge events. We are seeking an individual to help lead the development of this role during an exciting time for General Practice.
Within this position you will directly improve patient care and support the efficient running of a new model of general practice. You will become the practice expert in medicines optimisation and have input into the following areas but not limited to:
* undertaking structured medication reviews,
* improving medicines optimisation and safety
* running practice clinics
You will be part of a group of PCN pharmacy professionals. You will have the opportunity to access and complete NHS Englands accredited training pathway that equips you to practice and prescribe safely and effectively in a primary care setting
Main duties of the job
To improve patients health outcomes and the efficiency of the Primary Care team by providing direct, accessible and timely Clinical Medicines expertise, by working as part of a Multi-Disciplinary Practice Team. To pro-actively transfer workload relating to medicines optimisation issues from other clinical staff to improve patient care, safety, thus freeing up clinical staff to spend more time on other clinical care:
* To provide relevant support to the primary care team
* To promote safe, evidence based, cost effective prescribing in line with CCG requirements
* To provide expert pharmaceutical advice to Practices and the Medicines Optimisation Team
* To provide prescribing support to the designated Practices and team members in areas such as
* Medication reviews and follow up
* Implementation of prescribing changes
* Review of repeat prescribing
* Audit
* The post holder will provide leadership on quality improvement and clinical audit, as well as managing some aspects of the Quality and Outcomes Framework.
This role is pivotal to improving the quality of care and operational efficiencies so requires good motivation and passion to deliver an excellent service within a General Practice environment.

Job responsibilities
Ensure delivery of the medicines management aspects of the PCN DES contract, local Place prescribing scheme and focusing on the medicines aspects of QoF domains.
To develop quality improvement for medicines management across the patient demographic with a primary focus on complex and problematic polypharmacy and medication safety. The role will be key in identifying areas for improvement, as well as initiating and managing change.
Support the pharmacy team with clinical medication reviews, chronic disease management and high risk medicines. Review and implement processes to increase the safety and quality of prescribed medicines.
Key Duties and Responsibilities
* Provide leadership on quality improvement and clinical audit
* Support the delivery of QoF, incentive schemes including IIF and local prescribing scheme, QIPP and other quality or cost effectiveness initiatives
* Promote safe, evidence based, cost effective prescribing in line with ICB requirements
* Provide expert pharmaceutical advice to Practices and Medicines optimisation team
* Improving medicines optimisation and safety, including completion of prescribing audits in areas directed by GPs, feedback the results and implement changes in conjunction with the practice team
* Undertake clinical medication reviews with patients and produce recommendations for nurses / GPs with regard the prescribing and monitoring of patients
* Respond to medication related enquiries from GPs and other practice staff
* Support the repeat prescribing system and acute prescription requests
* Identification of cohorts of patients at high risk of harm from medicines through EMIS searches
* Analyse, interpret and present medicines data to highlight issues and risks to support decision making
* Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance
* Work with the general practices teams to ensure practices are compliant with CQC standards where medicines are involved
* Review medications for newly registered patients
* Reconcile medicines following patients discharge from hospital or clinic appointments
* Discuss with patients, if appropriate, any changes in treatment from discharge or clinic appointments
* Actively work toward developing and maintaining effective working relationships both within and outside the practice and locality. Foster and maintain strong links with all services across the locality. Demonstrate ability to integrate general practice with community and hospital pharmacy teams
* Liaise with all stakeholders in prescribing related matters to ensure consistency of patient care and benefit
* Support public health campaigns inline with practice / PCN needs. Provide specialist knowledge on public health programmes available to the public
Person Specification
Qualifications
* Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council(Essential)
* Enrolled or qualified from an approved 18 month training pathway or equivalent (For example the CPPE Clinical Pharmacist training pathways)(Essential)
* Hold or working towards Independent Prescriber qualification (Desirable)
Experience
* -Broad knowledge and understanding of the current NHS policy agenda
* -Knowledge of IT systems and a data-driven approach, looking at evidence and information available to support understanding of challenges and identify possible solutions
* -Knowledge of the regulatory and contractual frameworks for contracts within primary care, for example GP Network Contract DES, the standard GP contract and QoF
* -Excellent interpersonal, influencing and negotiation skills
* -Excellent written and verbal communication skills
* -Organisation skills with the ability to constructively challenge the view and practices of managers and clinicians
* -Effective time management (planning and organising). Be flexible and able to manage sudden and unexpected demands
