Spencer Clarke Group are seeking a Business Support Co-Ordinator for a Local Authority Client based in Berkshire. In this role, you will coordinate hospital discharges, manage referrals, and work closely with health and social care professionals to ensure a smooth transition for service users into appropriate care pathways. Key Duties: Manage referrals via the D2A pathway, screen cases, and ensure a smooth transition from hospital to appropriate care services. Liaise with health and social care professionals, maintain the hospital hub inbox, and support service users and families. Update and maintain hospital databases (EPIC & LAS), track patient flow, and ensure accurate record-keeping. Provide administrative support, adhere to policies, adapt to changing demands, and contribute to service enhancements. Qualifications and Experience: The successful candidate will have the following skills / experience: Experience in adult social care, hospital discharge systems, and the D2A pathway. Managing referrals, coordinating care pathways, and liaising with multidisciplinary teams. Proficiency in using systems like EPIC & LAS for tracking and reporting. Handling complaints, working with diverse communities, and managing multiple priorities under pressure.
