Job summary An exciting opportunity has arisen for a nurse to join our South Waveney Primary Care Home team delivering high quality nursing care. The team provides care at home to patients registered with GP surgeries in Waveney. The PCH team works closely with GP's and other members of the multidisciplinary team to provide an excellent standard of person centered nursing care to adult patients in their own homes, residential care or clinic settings, supporting patients to self-manage whenever possible. The service uses SystmOne as a clinical IT system to support mobile working. The successful candidate will be accountable for the effective management of a caseload, ensuring the delivery of high standards of evidence-based nursing care.
