Senior Practitioner Fast Track *ICS candidates only*

The Fast Track team is seeking an experienced registered nurse or allied health professional motivated by providing outstanding person-centred end of life support.
We have a full-time Senior Practitioner position available within our Fast Track team as a 12-month fixed-term contract. We would also welcome applications from individuals with prior agreement from their organization for a secondment opportunity.
The role involves working within the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care to determine eligibility for Fast Track continuing healthcare. You will be responsible for the review of completed Fast Track applications, liaison with referring professionals, and close partnership working with system-wide services. You will also liaise with hospital trusts regarding applications for patients awaiting discharge. You will participate in the commissioning and reviewing of packages of care to ensure they are safe and sustainable in supporting an individual's assessed needs. You will also be responsible for reviewing eligibility should someone's needs change. The role also provides an opportunity to assist in the delivery of Fast Track training to colleagues across the system to aid understanding of the National Framework and support high-quality and timely applications.

Main duties of the job
The successful candidate must be able to work collaboratively with Health and Social Care practitioners, patients, families, and carers, ensuring a responsive and high-quality service. You will be empathetic, honest, and have advanced communication skills with the capacity to hold compassionate or difficult conversations. You will have a flexible approach and be able to work autonomously. A knowledge of the National Framework and/or experience of working in end-of-life services is desirable.
You will have a sound understanding of safeguarding principles and have knowledge of the Mental Capacity Act and the Mental Health Act.
The All Age Continuing Care team is a supportive team with a wealth of skills and expertise providing a robust peer support and development programme in line with the CHC Competency Framework. There is also a strong focus on the wellbeing of staff.
The service operates Monday to Friday between the hours of 9-5, and we offer a home/office hybrid working and flexible working opportunities.
Due to the nature of the role, there is a requirement to travel, and a full driving licence is essential. The successful candidate will also be subject to an Enhanced DBS check.

The post holder will play a key role in supporting the Personal Health Commissioning Service. Working within the remit of the National Frameworks for Continuing Healthcare, the post holder will deliver an effective and competent level of service and consistently deliver a client-focused service.
The post holder will be responsible for assessing and reviewing patients, ensuring The National Framework for NHS Continuing Healthcare and Funded Nursing Care is implemented and complied with, according to the principles and processes. Ensuring value for money that meets the needs of the individual and delivery of best practice healthcare.
Person Specification
Qualifications
* Educated to degree level in relevant subject or equivalent level of experience working at a similar level in a specialist area, ideally Personal Health Commissioning
* Nursing qualification or Allied Health Professional E.g. Occupational Health, Physiotherapy etc. and registered with a professional body
* Knowledge of NHS Personal Health Commissioning and associated procedures and practices, underpinned by theoretical knowledge of the National Framework for NHS Personal Health Commissioning and NHS-funded Nursing Care and Children's and Young People's Continuing Care
* Evidence of Continued Professional Development
* Extensive knowledge acquired through experience and training plus further knowledge of the Continuing Healthcare process
* Good understanding of Personal Health Commissioning
* Experience of working in a health environment
* Negotiation and conflict management skills and the ability to influence
* Previous experience of caseload management
Skills, Capabilities and Knowledge
* Demonstrated capability to act upon incomplete information, using experience to make inferences and decision making
* Demonstrated capabilities to manage own workload effectively and make informed decisions in the absence of required information, working to tight and often changing timescales
* Ability to analyse very complex cases where material is conflicting and drawn from multiple sources
* Evidence of continued professional development
