They will coordinate the delivery of a safe and well-planned domiciliary care service that is service user focused and person-centered way. Hours: Monday – Friday plus weekend on call as part of a rota. The successful person will produce staff rotas and be capable of dealing with rota issues to ensure all support is delivered in all instances as well as providing Support Workers with the necessary information relating to the people using the service to enable them to deliver the best service. You will call each person once every month to discuss their support. Care being delivered must be checked on an ongoing basis throughout every day to make sure any gaps are covered and the Support Workers have attended their allocated sessions. Knowledge of the local area is essential as the Care Coordinator will be responsible for creating runs that minimise travel and maximise staff time. You will proactively liaise with external agencies such as Hospitals, GPs and Social Services, CMHTs, as well as family members and advocates in order to assess people for the service and review existing users. A full drivers licence and access to a vehicle is essential for this role along with good IT skills, excellent communication skills and the ability to work within a fast paced and ever changing environment. This role is ideal for you if you have the ability to work on your own initiative and prioritise your workload, good listening and communication skills, display the ability to understand and follow policies and procedures as well as good writing skills to complete assessments, risk assessments, care plans and onward referrals as necessary. Responsibilities Ensuring that people are supported with social and physical activities as well as basics such as meal preparation and prompting to maintain self-esteem and dignity § Ensuring people are assisted to get ready on a morning, go to work or University, do household tasks such as cleaning and cooking, budgeting, do social activities and attend appointments Monitoring individuals’ support to manage their health conditions compliance with medications Supporting the weekend on call rota to ensure the continuation of care and support on weekends Investigating and responding to complaints, accidents, incidents and safeguarding concerns. Completing quality checks to ensure the service is delivered in keeping with the key lines of Enquiry. Ensuring staff supervision, training and development. Skills and experience Your induction will include necessary training such as health and safety, first aid, medications and moving and handling. We also offer additional specific training around autism awareness, mental health conditions, communication skills and working with people who have experienced trauma. Good English and number skills such as GCSE in English and maths along with a social care qualification are beneficial but are not a barrier and we can support you to achieve these during your employment. At least 2 years’ experience of managing staff and working with vulnerable adults.
