Key Responsibilities
* Provide advice and support to individuals, empowering them to live independently.
* Conduct assessments to identify support needs and develop action plans accordingly.
* Build strong relationships with individuals, families, and other professionals to coordinate and deliver effective support.
* Ensure the safety and wellbeing of service users by adhering to relevant policies and procedures.
* Maintain accurate and up-to-date records of interactions, assessments, and plans.
* Promote access to community resources, including housing, healthcare, and financial assistance.
* Conduct home visits and attend meetings as required.
Requirements
* Full UK Driving License and access to a vehicle (essential).
* Experience in a similar role within a housing, social care, or community support setting.
* Excellent communication and interpersonal skills.
* Strong organizational and time-management abilities.
* Ability to work independently and adapt to a flexible schedule.
