You will be joining our existing and well-established Heart Failure Team within a community setting and with close links with our acute partner, Cardiac Rehab and Palliative Care Team. The team is also well supported by a consultant cardiologist and administratorsIn addition to managing a caseload of heart failure patients in their own homes, virtually or in a clinic environment, you will support the Heart Failure Virtual Hospital, and support the implementation of innovative pathways to transform the way Heart Failure patients receive care across West Hertfordshire. Main duties of the job You will be an experienced and committed nurse, paramedic or clinical pharmacist with cardiac experience who is motivated and energised to take part in innovation to change the way services are delivered to improve patient outcomes.The role involves working closely with GP's, other members of the integrated community teams, and with social care partners.
