Main duties of the job
As a skilled Care Coordinator, you'll already be used to managing a caseload of care packages and understand the varied nature and fast pace of the role, along with all elements of associated care delivery.
You’ll apply your gained communication and relationship-building skills to effectively manage rotas for your care teams, respond efficiently to any day-to-day changes, and act as the first point of contact for service users, their families, and other healthcare professionals.
No matter which care setting you've obtained your Care Coordination experience in, you’ll be aware that no two days are the same, so you’ll already be able to evidence your adaptability and aptitude to work well under pressure, using your initiative and thinking on your feet.
This is an office-based role, Monday to Friday, 9am to 5pm, but at times, travel across the East & South East region will be required. Therefore, applicants must hold a Driving Licence and have access to a vehicle.
You must also be committed to supporting the out-of-hours on-call service, which is covered on a 1 weeknight per week and 1 weekend in 5 rotational basis across the team.
We're offering a competitive salary between £26,000 - £28,000 per annum, plus payment for on-call. There is also the opportunity to earn a bonus of up to £2,000 per annum.

Person Specification
Experience
* Previous Care Coordination experience is essential in order to be successful for this role.
* It would be advantageous if you have experience of working within the complex care sector; however, this is not essential to be successful for this role.
