Job Description

As a Care Coordinator, you will be the critical link between clients and care professionals, ensuring that every individual receives the right care at the right time. Your responsibilities will include:
* Scheduling and Planning : Managing care schedules efficiently, ensuring clients’ needs are met while supporting the well-being of care professionals.
* Problem-Solving : Thinking creatively and proactively to resolve day-to-day challenges, including last-minute changes.
* Communication and Negotiation : Working empathetically with clients and care professionals to build trust and negotiate solutions that benefit everyone.
* Team Collaboration : Supporting colleagues, responding to requests firmly but fairly, and maintaining a growth mindset to turn mistakes into learning opportunities.
* Organisation and Focus : Staying on top of tasks in a busy environment, prioritising effectively, and keeping distractions to a minimum.
Qualifications

* Has excellent organisational and planning skills, with the ability to think outside the box.

* Possesses strong negotiation skills and the ability to handle challenges with confidence.

* Has a good knowledge of Shropshire’s geography, helping to coordinate care efficiently.

* Communicates clearly and concisely, keeping discussions purposeful and efficient.
* Is flexible and adaptable, ready to meet the demands of a dynamic role.

Working Hours: Monday-Friday 8:30am - 4:30pm, with occasional out-of-hours visits and participation in an on-call rota.
