An exciting opportunity has arisen to join our Patient & Carer Wellbeing Service (PCWBS). The PCWBS provides a blended model of telephone support, outpatient and face-to-face services.
This is a part time role working 4 days per week. You will work at our Coventry Hospice and travel to our Warwick and Rugby sites when required.

Main duties of the job
Working closely with a multi-disciplinary team, you will coordinate the assessment, planning and evaluation of individualised, patient-centred care, with a holistic approach that is sensitive and responsive to the ever-changing needs of patients, families and carers.
We are looking for an NMC Registered General Nurse with:
1. ENB 998 or equivalent teaching and assessing qualification
2. Evidence of continual professional development
3. Significant post-registration experience in:
4. Needs assessment / care planning
5. Palliative and end of life care
6. Assessment and symptom management
7. Skills in communication and organisation

Because you will be working with patients and their families, we will carry out an enhanced DBS check including the adults' and children's barred lists.
As this role requires travel, you must be able to travel within Coventry and Warwickshire.

Job description
Job responsibilities
Under the supervision of the Service Lead / Senior Staff Nurse, the postholder will engage with patients and their families / carers living with a life-limiting illness based around what is important to them. The service endeavours to meet the holistic needs of patients - physical, psychological, social, and spiritual needs and support.
The postholder will also work alongside the Service Lead, Registered Nurses, Nursing Assistant, Volunteers, and the wider members of the multi-disciplinary team to meet the individual needs of patients who remain stable in their condition but require ongoing support and follow-up. The role is to facilitate and maintain the philosophy of the hospice and a palliative care approach and ensure that the Myton values are upheld at all times.
Contact will be mainly through telephone and/or virtual engagement and facilitating the PCWBS groups across the three hospice sites, which take place Monday to Friday.

Main Duties & Responsibilities
27. Engage and positively deliver, with the assistance of volunteers, individual and group activities of support and education aligned to the service, for patients and their families, regarding the various aspects of their health and wellbeing.
28. Provide compassionate, high quality and evidence-based care within the service, maximising the specialist palliative care and end of life skills of staff.
29. Demonstrate effective communication and work in partnership with patients, carers, PCWBS nursing team and other members of the multi-disciplinary team to ensure that care and support reflects individual needs. To alert the Service Lead / Senior Nurses of any concerns of crises or problems arising with a patient or family member / carer.
30. Proactively work in partnership with other providers externally to ensure that all information pertaining to the patient's condition and care needs are communicated effectively to ensure optimum use of resources and maximising opportunities for community engagements.
31. Demonstrate skills in conflict resolution and competent negotiation skills when dealing with difficult or challenging situations.
32. Act as a positive role model to others and ensure that practice of specialist palliative care is evidence-based.
Other Duties
33. Abide by the policies and procedures of The Myton Hospices, the guidelines and Code of Conduct of the Nursing and Midwifery Council and the regulations and standards of the Care Quality Commission.
34. Ensure the working environment is safe, clean and tidy and that all staff adhere and comply with the Infection Control Policy, dress code and all other Policies and Procedures.
35. Be responsible for own continued professional development to maintain up to date knowledge across the palliative care trajectory.
36. Be aware of the political and national influences affecting clinical care, such as NICE Guidance / End of Life Care Strategy.

38. Promote a culture where the reporting of concerns, incidents and complaints is encouraged and documented promptly.
39. In accordance with Data Protection, create and maintain accurate, contemporaneous patient records.
40. Complete documentation accurately and legibly in accordance with NMC guidance.
41. Maintain confidentiality using Caldicott Guidelines.
Training and Education
42. Be responsible for own continued professional development.
43. Attend appropriate training, development sessions and courses to meet own professional needs and ensure that the level of competence and specialist knowledge required for the role is maintained.
44. Maintain up to date evidence-based knowledge including an awareness of the national and local influences affecting palliative and end of life care.
45. Ensure own compliance with mandatory training and professional development requirements, maintaining records of training and development undertaken for self in preparation for NMC revalidation.

Infection Control:
All employees must be aware of infection prevention and control policies and are expected to follow them at all times. Any breach of infection control policies which places patients, visitors or colleagues at risk may result in disciplinary action.

Health & Safety :
