Attending multi-disciplinary teammeetings, creating detailed person centred support plans and risk assessments to ensurea safe delivery of service.Who will your manager be and what will you be responsible for?You will be:
* reporting to Lead Reablement Officer who is in turn responsible for your health and
safety, training and development.
* responsible and accountable for 20 FTE members of staff, acting in a supervisory
capacity, monitoring performance and supporting development.
* expected to lead and/or contribute to a range of projects and build relations with
both internal and external partners in order achieve specified outcomes but will not have direct authority over those involved.What will you be doing?To plan and review short term person centred support for people requiring short termreablement, preventing hospital admission and supporting hospital discharges.Irrespective of diagnosis, Senior Reablement Practitioner are to assist people to regain orretain skills to continue to live as they wish. The multi-skilled reablement officers are toplan the reablement with the person to ensure that reablement practitioners can performtasks of daily living like, self-care, cooking meals, washing, dressing, administeringmedication and getting about the home. By encouraging the promotion of self-care skills.Holistically plan the support individuals using a person-centred, strengths-based approachthat promotes and maximises independence and wellbeing. Support and enable serviceusers to improve or maintain their mobility through exercise and the use of mobilityappliances.Rebuild confidence and independence after a spell of illness, deterioration in health, ahospital admission, or an acquired disability. To monitor the physical and emotional wellbeingof all people within the START service in order to be aware of changes and to alertmulti agency colleagues as necessary.Ensuring wrap around support for the people within the service, Senior ReablementPractitioner's communicate directly with verbal, written and electronic documentation withmultiple health, and social care professionals. Such as, district nurses, pharmacists, GP's,social workers and clinicians with the rapid response team, to enhance communicationswith multi-agency working ensuring that the person is in the centre of the package of careand informs their decision making. The person will progress through their reablementjourney. Senior Reablement Practitioner's Involve the person in an exit procedure to reviewgoals achieved and to signpost to other services that are available including communitybasedactivities. Focused on the person's needs and how they impact on their wellbeingin line with the Care Act 2014Observe members of the team in practice to able to evidence their competence with safehandling of medication, general practice, moving and handling. Teach and develop theskills and knowledge to the wider team through observation and supervisionRespond and liaise with emergency services dealing with front line emergency situationswith 111, 999 and West Mercia Ambulance Service. Communicate with Shropshire Fireand Rescue service if safe and well visits have been identified and reported to the agencyRecord, complete and document risk assessments for the people within the service byconsidering risk levels and control measures whilst ensuring both staff and people withinSTART's service are safe.To oversee lone working reablement practitioners to ensure that they have the means tosafely administer, dispose, record and monitor effectiveness of medication to clients andensure stock items under your control are ordered and available. To ensure confidentialityand document record keeping is of a high standard. Whilst liaising with STARTManagement team for arising matters. This includes Warfarin and controlled drugs.Be a manual handling champion to comply with safe systems of work in accordance withDepartments Health and Safety policies and procedures by completing the relevant riskassessments within service user's homes.Be a trusted assessor. Through listening to individuals, identify barriers to theirindependence and provide effective solutions to removing those barriers. Identify anddemonstrate the use of products such as perching stools are designed to giveindependence and understand how non-routine solutions can also contribute to a person'sindependenceDirect and indirect Catheter care. Provide advice and guidance to the people requiringsupport in this area with effective record keeping of their personal progress record keeping.Recognise, be aware of any situations, incidents and report any concerns of person withcare and support needs in line Shropshire Council policy and procedure.There are significant clerical and administrative tasks related to this role using both IT andpaper-based systems.Observe members of the team. Teaching and development of skills and knowledge to thewider team through observation and supervision.To assist the START team in ensuring that individual and service performance targets aremet in line CQC regulations. * The post is permanent for 37 hours per week,
* Working a mixture of days, evenings, weekends and Bank Holidays 365 days
per year between the hours of 07:00-22:00
* Participation in a rota for the START out of hours service.

It is a'doing with' service, in contrast to traditional home care which tends to be a 'doing for'service. START Reablement services help people to retain or regain their skills andconfidence so they can learn to manage again after a period of illness.Rapid ResponseThis service aims to avoid unnecessary hospital admissions and safely support people intheir own home. This a joint partnership between Shropshire Community Health Trust andShropshire Council. Nurses, care coordinators and support workers, across bothorganisations work together to ensure a seamless service that provides a rapidassessment, treatment, and care program of support to stabilise and support people duringan acute episode of ill health
Annual leave entitlement is a pro rata flat rate scheme of 200 hours (27 days) annual leaveplus bank holidays, with five days extra awarded to those staff with five years localgovernment service giving an entitlement of 237 hours (32 days). Two days of anemployee's allowance (pro rota for part time staff) must be taken at Christmas for anypotential Christmas closures. Employees who work in a building/service which is requiredto open over the Christmas period, the two days leave (pro rota) can be carried over intoyour next leave year but must be used by the end of March.The appointment is subject to one month's notice in writing on either side.The appointment is subject to six months' satisfactory probationary service during whichtime the notice period will be one week on either side.Smoking is not allowed in Council buildings, in Council vehicles or in any Council place ofwork.It is a condition of your appointment that you provide a suitable vehicle for the performanceof your duties and that this is readily available for use during normal working hours.
