Job overview
We are seeking a highly motivated nurse to form part of the Discharge Lounge Service within the Trustwide ISU. Working 23 Hour week. The Sister/Charge Nurse will support the Clinical and Operational leads to manage and lead the Discharge Lounge. Excellent working knowledge of discharge planning and patient flow in acute services essential.
* To work as part of the clinical team to ensure seamless delivery of service.
* To clinically co-ordinate the unit to maintain flow and patient journey.
* Support with identifying patients, transferring patients around the hospital. Complete handover process, complete discharge checklist (e.g. CPS, TTAs etc)
* To participate in the assessment of care needs of the clients and the development, implementation and evaluation of care, promoting high standards, demonstrating a compassionate nature and promoting patient dignity.
* To participate as a skilled member of the multi-disciplinary team
* Promote good interdepartmental relationships.
* To act as clinical advisor on nursing issues to ensure high standards of care are maintained, utilising evidence-based practice.
* To participate in direct care and facilitate sound discharge planning.
Main duties of the job
* To ensure high standards of care are given in relation to agreed standards, policies and procedures of Torbay and South Devon NHS Foundation Trust.
* To participate in teaching and facilitating a learning environment to ensure clinical staff on the ward have the skills/knowledge and support – identified by regular staff appraisal and annual personal development plan.
* To clinically manage the discharge lounge effectively and safely.
* To assist the discharge lounge coordinator and Operational Manager in the management of unit resources – budget, staffing, annual leave etc as required.
* Ensure adequate skill mix to meet the needs of the clinical area.
* To be proactive with transfers and discharges, ensuring they are safe and discharge is at the earliest opportunity.
* To ensure staff are aware of agreed standards of practice and participate in pursuing agreed units’ objectives.
* To assist in ensuring new nursing procedures, research and initiatives are evaluated, and where appropriate incorporated into clinical practice.
* Act as councillor and advisor to junior staff and learners on nursing issues.
* To undertake appraisals and personal development planning for clinical staff.
* To participate in the MDT and ensure necessary discharge documentation is completed in a timely manner.

* Registered Nurse with current NMC Pin number.
* Proven relevant experience at Band 5.
* Evidence of continual professional development.
* Discharge knowledge and experience.
* Post graduate diploma level education or equivalent.
