Job summary We're looking for Community Nurses to join our Bath Twilight community Nursing Service with a base at St Martin's Hospital, Bath to help us continue to change things for the better. The community nursing team works across 17:00 - 22:30pm over 7 days a week. Come and join our friendly and supportive twilight team nursing people in their own homes in and around Bath. This role is perfect for childcare and family life, working independently in the evening with an experienced healthcare assistant carrying out a variety and interesting array of nursing tasks. This is a part time position for 5.5 - 11 hours per week. Main duties of the job Within this role, you will: responsible for contributing to the holistic assessment, planning delivery and evaluation of care to patients within their own homes. implement, monitor and maintain high standards of care delivery at all times. care for patients and their families in the way that you believe is best deliver our high service standards, sharing best practice, and actively improving the way we work. End of life care, ensuring patients are comfortable at end of life including stat doses and syringe drivers.
