We are looking to recruit an enthusiastic and highly motivated Practice Nurse to join our small friendly and supportive team in a 4000 list SystmOne practice.
Ideally, we are looking for someone who has previous experience in Primary Care and who is trained in dealing with Chronic Disease Management, Cervical Screening, and Childhood Immunisations.
We are looking to cover approx. 30 - 35 hrs but would be willing to consider full or part-time for the right candidate.
The post holder will be an experienced nurse acting within their professional boundaries.
You will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures.
You will be responsible for the clinical nursing care of the practice population. You will receive referrals from the general practitioner and nurse practitioner and will be called upon to advise, support and treat patients who attend the surgery. You will undertake the delivery of nursing care in general practice as a self-directing practitioner, working with the GPs to meet patient needs as appropriate.
Main duties of the job
* Cytology
* Removal of Sutures / post-operative care
* Urinalysis
* ECGs
* Chronic disease management in line with QOF requirements
* Venepuncture
* Ear syringing
* Routine immunisations/Childhood immunisations
* Chaperoning and assisting patients where appropriate who are being examined by another clinician
* Requesting pathology tests, for example urine culture, swabs
* Following agreed clinical protocols with referral to GPs as appropriate
* Maintaining GP and Nurses rooms, stocking and rotating items as required
* Maintain general tidiness and cleanliness of nurses and treatment rooms
* Ordering of Health Promotion Literature
* Ordering of stock
* Ordering of vaccinations to maintain stock levels
* Maintain and embed robust infection control measures.
* Participation in administrative systems in the Practice
* At all times there will be a need to maintain accurate records
* A duty to advise the Practice Manager of potential problems or errors within the range of assigned tasks
* Attend and participate in any Practice meetings when required.
* On occasion there may be a requirement to undertake home visits

As part of the Eston PCN, we offer enhanced access, and attached staff including physiotherapist, podiatrist, health and wellbeing coach, clinical pharmacist, care coordinator, mental health support, and social prescribers. Everyone within the team and wider PCN team is supportive, friendly, and approachable.
Job responsibilities
* To develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practice
* To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice
* To develop, implement and embed health promotion and wellbeing programmes
* To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing
* To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients' health
* As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF
* As required, to diagnose and manage acute and chronic conditions
* To prescribe medication for patients in accordance with PGDs and own scope of practice
* To review patient medications to enhance compliance
* To provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages
* To request pathology services and process pathology results as required
* To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
* To maintain accurate clinical records in conjunction with extant legislation
* To ensure read codes are used effectively by all members of the nursing team
* To ensure the appropriate maintenance and use of chronic disease registers
* To ensure compliance with the practice chaperone policy
* To assist GPs with minor surgery when required
* To prioritise health issues and intervene appropriately
* To support the team in dealing with clinical emergencies
* To recognise, assess and refer patients presenting with mental health needs
* To implement vaccination programmes for adults and children
