Qualifications
Essential criteria
* Foundation Degree in Health & Social Care
* NVQ Level 3 in Care
* Experience of working within a community team with a high level of autonomy
* Current or prior experience of working in health, therapy and/ or social care setting
* Excellent organisational skills
* Communication and interpersonal skills
* Ability to work on own initiative and prioritise workload
* Understanding of evidence-based practice and up to date knowledge of relevant literature
* Evidence of clinical skills competencies
* Ability to demonstrate empathy and be sensitive to diverse individual needs
* Ability to manage own case load and work without direct supervision
* Ability to work as part of and contribute to a multidisciplinary team
* An understanding of the importance of confidentiality
* Flexibility and ability to adapt to changing needs of clients, team and service needs
* Ability to work safely and reliably in both unit setting and clients' homes.
* Knowledge of therapy skills and practices including equipment
* Knowledge of health and social care.
* To use clinical supervision appropriately, utilizing reflection and critical appraisal of own performance
* Clean Driving licence and drives own car
Desirable criteria
* Health related qualification such as Clinical skills.
* IT qualification or recent in-service training.
* NVQ Level 4 in Care.
* Diploma in health and social care.
* Experience of working within rehabilitation/district nursing
* Previous experience of working with adults with complex needs.
* Experience of working with people with chronic/progressive disease.
* Experience of working with people/families and carers who may be distressed or depressed.
* Information technology skills
* Up to date professional portfolio
* Effective organisational and administration skills
* To motivate and enable individuals
* Demonstrate a knowledge of National guidelines influencing the care of older people
* Health promotion/ Champion.
