We are seeking clinically astute, motivated, enthusiastic Advanced Nurse Practitioners to join our well established, experienced and friendly team. You will work flexibly across the Primary and Urgent Care locations to deliver timely and quality care, working closely with the Lead Nurses to manage and provide exceptional leadership to the nursing team. This is a valuable opportunity for Advanced Nurse Practitioners to work in an established but ever evolving service in which they will further develop and practice their nursing, clinical and leadership skills and contribute to its development.
Wirral Walk in Centres currently operate 7 days a week 8am-10pm, including weekends and bank holidays. The Urgent Care Centre offers the opportunity for Staff to work outside of these hours to enhance their work life balance. Twilight shifts will be available to support working parents with childcare.
You will be working within a caring, compassionate team who truly put the patients at the forefront of everything they do. You will also be part of a ground breaking new way of working between primary and community care providers, working towards removing boundaries between these services.

Main duties of the job
1. To assess, plan, implement and evaluate programmes of evidence based nursing & clinical care.
2. To provide expert advice and clinical practice.
3. To participate in the education and development of self and other staff.
4. To work collaboratively and co-operatively with clinical colleagues to develop services and quality of care delivered.
5. To autonomously assess, diagnose and treat children and adults who attend the Walk-in Centre with undifferentiated, undiagnosed conditions.

Job responsibilities
Clinical
1. Perform a comprehensive clinical assessment and evaluation of patients condition.
2. Develop and implement individual care packages for patients based on clinical findings.
3. Evaluate care delivery according to changing health care needs.
4. Interpret clinical findings to ensure most appropriate care is delivered.
5. Collect, collate, evaluate and report patient information, maintaining accurate records.
6. Work in partnership with patients, carers/relatives in the planning and delivery of care.
7. Work collaboratively with other professionals and agencies to ensure patients needs are met especially in the relation to on-going care needs and discharge arrangements.
8. Provide venepuncture for patients who have difficulty accessing established clinics.
9. Provide wound care management for Walk-in Centre patients.
10. Provide a range of treatments using patient group directions or nurse prescribing following clinical assessment and diagnosis.
11. Establish and maintain effective communication with patients and carers/relatives.
12. Contribute to service redesign.
13. Provide clinical support for junior staff.
Managerial/Leadership
1. Provide specialist, expert clinical knowledge to clinical colleagues, patients and carers/relatives.
2. Monitor housekeeping issues, maintenance of stock levels, ordering stock, ensuring faulty or non-working equipment is reported and repaired.
3. Participate and contribute to clinical supervision of nursing colleagues on a group or one to one basis.
4. Provide mentorship for new and junior staff.
5. Participate in the development of services and assist with the implementation and evaluation of change.
6. Monitor health safety and security of self and promote best practice in the departmental area.
7. Supervise the work of staff nurses, auxiliary nurses and bank staff.
8. Develop own broad and expert clinical knowledge and participate in the development of others clinical knowledge, in conjunction with Nurse Consultant / Professional Development Nurse.
9. Ensure own compliance with regard to mandatory training and NMC requirements.
10. Review and apply existing evidence based practice guidelines for care.
11. Undertake further education at higher level to promote growth and development of clinical skills and knowledge.

1. Insure compliance with policies, procedures and clinical guidelines for self and others, participate in the development of same.
2. Continually monitor standards of care and participate in the improvement of care, through benchmarking, audit and research.
3. Promote patient and public involvement activities in the specialist area, leading to service improvements.

Person Specification
Qualifications
* Registered with the Health Care Professionals Council.
* Degree/Diploma in relevant clinical area.
* MSC Advanced Practice.
* V300 Prescriber.
Experience & Knowledge
* Substantial experience of Community Services within Urgent and Primary Care and working clinically within those services.
* Experience in presenting and leading groups.
* Experience of working collaboratively with other agencies and professions to deliver change.
